Surgical Plan (After Pharma Optimization, 18+ due to parental restrictions)

Timing: Start ortho now, major surgery after major growth spurt but before full maturity if severe.

  • Orthadontics first (Immediate): Braces/Invisalign + possible palate expander (maxillary expansion for better zygos/lip support and airway). 12–24 months. Addresses upper lip, lip width, and sets up for jaw surgery.
  • Jaw Surgery (BSSO + Lefort I + Genioplasty):
  • Advance mandible (chin/jaw projection for severe recession).
  • Maxillary advancement/impaction for lip support, width, and midface.
  • Sliding genioplasty for chin length/width.
  • Can add 8–15mm projection easily — transforms profile dramatically.
  • Zygomatic/Malar Implants or Osteotomy: For flat/bad zygos — custom implants or bone cuts to widen/highlight cheekbones.
  • Brow Ridge / Forehead: If still deficient post-hormones, custom brow bossing implant or fat grafting.
  • Liposuction / Buccal Fat Removal: For persistent chubby face (after hormones/diet).
  • Canthoplasty+Upper eyelid fat graft.
  • Subnasal Lip Lift and a Lateral Commissuroplasty
  • Infra Orbital Fat graft for under eye support

Well, I'd say ur outline is directionally strong and ok, but it needs to be made more conservative, staged, and anatomy first if ur goal is a stable, peak result rather than a “stacked” look that can age badly the biggest correction is this: prioritize skeletal balance and airway/occlusion first, then only add soft tissue procedures that still make sense after swelling, settling, and orthodontic decompensation etc etc

Mirin who r u getting implants with? I can give some recommendations for surgeons and orthodontists... (for example Eppley makes great custom IOM implants but his jaw implants are shit and could be done cheaper by a normie surgeon/designer in turkey)

u can pm ur face if u want but its hard to know from here i would be careful about choosing the type of impaction you get on the maxilla carefully. Also 15mm is hard to get with normal people i would set the limit at around 8 mm for maxilla max and 12 mm for mandible max. even less with ccw. also needless to say wait until bimax+ genio fully heals before planning implants and soft tissue surgeries (except for fat grafts you can do those at 18) because the earlier you get the fat the better you will age. Also istead of getting a zygo implant i would get an infraorbital-malar implant which encompasses forward projection too, along with the zygomatic width.
